A form for the library to use to request assistance from IT with loaner laptops. Use primarily for broken laptops, inventory updates, laptop software updates, laptop returns, and to request additional laptops from IT to fulfill loaner requests. This form is intended solely for managing and reallocating existing loaner laptops and should not be used to request the purchase of new devices.
