  • Use this form to notify Technology Services of offboarding requests for separating employees, contractors, volunteers, and student workers.

  • Human resources typically submits requests for all offboarding for separating employees through the request link to the right. 

  • Supervisor or designee typically submit requests for contractors, volunteers, and student workers through the request link to the right.

Benefits

  • Timely notification allows for IT provided access to be removed. For separating employees notification is also sent to finance, security and facilities and the web and media manager.

  • Helps to manage resources allowing the return of technology equipment for updating and redistribution for future use.
